Your First Visit
| Your first visit will normally be scheduled with our hygienist
and doctor. It will consist of a complete examination (including any
needed radiographs) and a comprehensive treatment plan. If your tissues
are healthy at this visit, a regular prophylaxis (cleaning) will be
scheduled. If you have a more complex periodontal condition, it may
require multiple visits with our hygienist to properly treat you.
Please assist us at the time of your initial visit to the office by providing the following information: |

| Any current x-rays from your previous dentist can be very helpful in our diagnosis. If these are not available, we will take the necessary radiographs for your examination. Your insurance company may or may not cover these if you have had recent radiographs elsewhere. If patient registration and medical history forms were mailed to you, please bring them completed, or you may arrive ten minutes early and fill them out at our office. If you have dental insurance, please bring any forms or insurance cards with you. Please note: a parent or guardian must accompany all patients under the age of eighteen so that we may discuss treatment plan options. |
